Background Medical ethics deals with the ethical obligations of doctors to their patients, colleagues and society. The annual reports of Sri Lanka Medical Council indicate that the number of complaints against doctors has increased over the years. We aimed to assess the level of knowledge, attitude and practice regarding medical ethics among doctors in three teaching hospitals in Sri Lanka. Methods A hospital-based cross-sectional study was conducted among doctors using a pre-tested self-administered, anonymous questionnaire. Chi Squared test, and ANOVA test (...) were used to identify the significance of association between level of knowledge and selected factors. Results Most doctors had a poor level of knowledge on medical ethics, with postgraduate trainees showing significantly higher level of knowledge. The average knowledge on medical ethics among doctors was significantly different between the three hospitals. Over 95% had a favourable attitude towards gaining knowledge and advocated the need for training. The majority indicated awareness of unethical practices. 24.6% of respondents stated that they get a chaperone ‘sometimes’ during patient examination while 3.5% never do. The majority responded that they never accept gifts from pharmaceutical companies in recognition of their prescribing pattern. 12–41% of doctors participated in the study acknowledged that they ‘sometime’ engaged in unethical practices related to prescribing drugs, accepting gifts from pharmaceutical companies and when obtaining leave. Conclusion Most doctors had a poor level of knowledge of medical ethics. Postgraduate trainees had a higher level of knowledge than other doctors. The majority showed a favourable attitude towards gaining knowledge and the need of training. Regular in-service training on medical ethics for doctors would help to improve their knowledge on medical ethics, as well as attitudes and ethical conduct. (shrink)

Epidemics that were probably influenza have been reported throughout recorded history. There were 13 fairly severe epidemics during the 18th century and 12 during the 19th century. Probably 8 of these 25 were influenza pandemics. In the 20th century there have been 4 pandemics (1918/19, 1957/58, 1968/69 and 1977) due to the emergence of new subtypes of influenza A virus. The great pandemic of 1918/19 caused an estimated 20 million deaths. Between pandemics usually there have been epidemics of varying severity (...) at intervals of one to three years and a trickle of sporadic cases every winter. The morbidity and mortality rates have varied greatly from epidemic to epidemic and from place to place during the same epidemic. Generally the morbidity has been lowest in people over 60 years of age, but, except for 1918/19, the mortality has been predominantly in old people. The epidemic behaviour of influenza has been so erratic and difficult to understand that there are still a few scientists who consider that extraterrestrial influences operate. These views are not taken seriously by most virologists but there are puzzling aspects of influenza that are not yet understood. (shrink)

Abstract:This essay examines the experiences of three 19th-century African American travelers to Greece—David Dorr (1852), Frederick Douglass (1887), and John Wesley Gilbert (1890–1)—using evidence from their letters, diaries, and published writings. The essay shows that although each traveler's unique personal perspective shaped his response to seeing the ancient sites and monuments of Greece, all three men responded most deeply to a site connected with Greece's Christian heritage: the Areopagus or Mars Hill, where according to 19th-century understanding the Apostle Paul had (...) spoken the words recorded in the Book of Acts. Under the impact of cultural, economic and legislative forces the traditional role of the university health center is changing. The academic health center is rapidly evolving from a relatively undifferentiated general hospital, primarily responsible for the education of undergraduate students of medicine, into a center of clinical research, caring for very specialized mixes of patients, and having as its primary educational mission the training of subspecialists. The nature of the forces responsible for this change are analyzed, and some of its (...) possible consequences for the definition of quality health care, for the shape of medical practice and medical education, and for the allocation of resources for health care are explored. This exploration suggests that the functional separation of the medical school from the highly technical, clinical research center is inevitable. This in turn may exacerbate tensions within medicine between the physician as scientist and as healer, between curative and preventive practice and between personal and public health. Although the effects of this separation on medical education remain obscure, it has already occasioned the most comprehensive review of the medical curriculum since the time of Flexner. Out of this is likely to develop a more generalist view of the education of future physicians. (shrink)

It is not surprising, therefore, that in successive ages Heraclitus has been held up to glory or obloquy as the teacher of a Flowing Philosophy eventuating in irrationalism and mysticism; as the inspirer of Stoicism, conflagration and all; as the avatar of Satan behind the Monarchian heresy (...) of Noëtus; as he who first dimly discerned the Grundeinsicht of Hegelianism; as a Fascist, a Marxist, a Nietzschean, an Existentialist. Our only channel of information about the world is the impact of external forces on our sensory surfaces. So says science itself. There is no clairvoyance. How, then, can we have parlayed this meager sensory input into a full-blown scientific theory of the world? This is itself a scientific question. The pursuit of it, with free use of scientific theory, is what I call naturalized epistemology. The Roots of Reference falls within that domain. Martin Buber is considered one of the 20th centuryes greatest thinkers and his contributions to philosophy, theology and education are testimony to this. His thought is founded on the idea that people are capable of two kinds of relations, namely I-Thou and I-It, emphasising the centrality of dialogue in all spheres of human life. For this reason, Buber is considered by many to be the philosopher of dialogue par excellence. After Buberes death the appreciation of his considerable legacy to the (...) various disciplines in which he had worked became rather muted, but was never completely forgotten. There is now a renewed and growing interest in Buberes thought, especially in his philosophy of education. This book brings together aspects of Buberes philosophy and educational practice, and explains their significance for peace dialogue and for conflict resolution, both between individuals and communities. Buber's philosophy of dialogue and views on education are pivotal in demonstrating the personal and social benefits of dialogical education as well as the dangers of non-dialogical education. This essay defends a reading of Hegel as a metametaphysical antirealist. Metametaphysical antirealism is a denial that metaphysics has as its subject matter answers to theoretical questions about the mind-independent world. Hence, on this view, metaphysical questions are not, in principle, knowledge transcendent. I hold that Hegel presents a version of metametaphysical antirealism in the Science of Logic because he pursues his project by suspending reference to all supposed objects of metaphysical theory as practiced before him. Hegel introduces reference in (...) his theory only by the self-reference of thought to itself in the Doctrine of the Concept. I motivate the relevance of Hegel’s approach to metaphysics by comparing it to Kit Fine’s (2017) concept of “naïve metaphysics.” I argue that Hegel’s theory results from a comprehensively naïve metaphysics. Being and Time is a methodologically complex work, combining hermeneutic, transcendental, phenomenological, and ontological strategies in a provocative and not-obviously-stable concoction. In this article, I focus on one strand of the methodological puzzles raised by Heidegger’s undertaking: the problem of warranting the modal claims that occur frequently in the course of Heidegger’s project. In a number of crucial passages, we are told that one or another trait of Dasein is necessary, or that some ontic feature of Dasein would not be (...) possible were it not for some deeper ontological feature. I undertake to determine the logical form of these doctrines, and to consider what kind of evidence might suffice to establish them. I draw on Heidegger’s complex debt to Dilthey in proposing an interpretation of the notion of an existeniale, and I critically assess Taylor Carman’s treatment of Heidegger’s project as an extension of Kantian transcendental strategies. In the end, I argue, much comes to turn on one’s account of the semantics of Heidegger’s central term of art: “Dasein.” I identify shortcomings in two possible approaches to this problem: one takes the extension of the term to be antecedently fixed; the other fixes the meaning of the term by specifying its intension. I then explore an alternative semantics for “Dasein” under which the modalized doctrines of Being and Time can be considered de re necessities. All three of the semantic models that I consider here remain highly schematic – cartoons rather than fully elaborated portraits – and I do not mean to suggest that any of the three would suffice to capture the enormously complex semantic structure of Heidegger’s undertaking. Nonetheless, I argue that the third semantic model enjoys certain demonstrable advantages over the other two, both for mounting a defense of Heidegger’s modal propositions and as a schema for mapping the text of Being and Time. It also allows us to frame a challenge that any fully adequate semantic interpretation of Heidegger’s text would have to meet. (shrink)

Background Fifteen years have passed since the outbreak of severe acute respiratory syndrome in Hong Kong. At that time, there were reports of heroic acts among professionals who cared for these patients, whose bravery and professionalism were highly praised. However, there are concerns about changes in new generation of nursing professionals. Objective We aimed to examine the attitude of nursing students, should they be faced with severe acute respiratory syndrome patients during their future work. Research design A questionnaire survey was (...) carried out to examine the attitude among final-year nursing students to three ethical areas, namely, duty of care, resource allocation, and collateral damage. Ethical considerations This study was carried out in accordance with the requirements and recommendations of the Central Research and Ethics Committee, School of Health Sciences at Caritas Institute of Higher Education. Findings Complete responses from 102 subjects were analyzed. The overwhelming majority (96.1%) did not agree to participate in the intubation of severe acute respiratory syndrome patients if protective measures, that is, N95 mask and gown, were not available. If there were insufficient N95 masks for all the medical, nursing, and allied health workers in the hospital (resource allocation), 37.3% felt that the distribution of N95 masks should be by casting lot, while the rest disagreed. When asked about collateral damage, more than three-quarters (77.5%) said that severe acute respiratory syndrome patients should be admitted to intensive care unit. There was sex difference in nursing students’ attitude toward severe acute respiratory syndrome care during pregnancy and influence of age in understanding intensive care unit care for these patients. Interestingly, 94.1% felt that there should be a separate intensive care unit for severe acute respiratory syndrome patients. Conclusion As infection control practice and isolation facilities improved over the years, relevant knowledge and nursing ethical issues related to infectious diseases should become part of nursing education and training programs, especially in preparation for outbreaks of infectious diseases or distress.
